For older people to be considered for admission to a long-term facility, the New York State Department of Health requires a comprehensive evaluation of their health care needs and functional abilities, and completion of a Patient Review Instrument (PRI) assessment. A specially trained ElderServe nurse will schedule a visit to a client's home to explain and conduct this assessment at a time convenient for the client and his or her family.
