Social Model Adult Day Care Services

Social model adult day care is a setting where seniors can gather for structured socialization and recreation in a supervised environment. A caregiver may choose a social program as a next step in the care of a loved one who may be in the early stages of dementia or declining in cognitive ability. Our programs allow caregivers to have much needed respite while assured that their loved one is in a monitored environment from  one to six days a week.

The Hebrew Home at Riverdale offers three unique social programs  serving the Upper West Side of Manhattan, Upper East Side of Manhattan and Westchester County All programs are collaborations with various local social service and medical facilities serving older adults with memory problems. Social, recreational, therapeutic and educational activities are offered throughout the day, including exercise classes, arts & crafts, discussion and reminiscence groups and music. Group activities are designed to meet the individual needs and abilities of each member. Services are provided in an attractive, home-like setting, which is handicap accessible.

Door- to- door transportation is available. Nutritious meals and snacks are served.

ElderServe on the Palisades – Day Session

ElderServe on the Palisades is a medical model adult day health care program that provides a viable option for seniors and their caregivers, who choose to remain within the community.  Elder Serve on the Palisades provides a medically monitored program in a safe, stimulating, and fun-filled setting located on the park like grounds of the Hebrew Home at Riverdale. Seniors experience socialization, recreational activities, exercise, alternative therapies and community outings. Caregivers get the respite they need, while assured that their loved one is under the supervision of a trained clinical team, which includes registered nurses, registered dietitian, social workers, occupational and physical therapists, and recreational facilitators.

ElderServe at Night – Overnight Session

ElderServe at Night is an adult day health care program that operates from 7:00pm – 7:00am.  It is  the only overnight adult day healthcare program in the country and is designed for people living at home who require constant supervision during the night.  A medical model program serving a population suffering from sleep disturbances associated with various dementias, it is unique in providing participants a host of activities throughout the night. 

This session caters to the confusion and sleepiness that some seniors may encounter as the sun goes down, a phenomenon known as “sundowning.”  For the caregivers the program provides peace of mind and allows them to get a restful night sleep knowing that their family members are well taken care of and in good hands participating in  a medically supervised evening of socialization, recreation, and fun.  

Services Provided

Both day and night sessions provide assistance with daily living skills, such as bathing, grooming, and eating. We serve a medically diverse senior population that includes people who are physically frail or have a dementia related diagnosis.  This program operates seven days a week, 365 days a year from 9:00am – 3:00pm (day session) and 7:00pm – 7:00am (overnight session). Door-to-door transportation is available. This service may be used from one to seven days a week depending upon the senior’s primary care physician approval. Nutritious meals and snacks are served each day and night.

ElderServe Long-Term Home Care Services

The ElderServe Long-Term Home Health Care Program is designed for persons who choose to remain in their own home although eligible for placement in a long-term care facility. The program provides a full range of health care, social and supportive services, including medical management, nursing services, rehabilitation therapies and personal care in a client's own home. Additional care options may include housekeeping, home maintenance, Adult Day Services and the Personal Emergency Response System.

Care Management
A Case Manager, who is a Registered Nurse, plans, supervises and coordinates all Long-Term Home Health Care services. When a referral is made, a nurse visits the home to conduct a comprehensive assessment of the client's needs. the plan of care is developed in conjunction with the client, their physician and family. Acting as a liaison with the client's physician, and all other ElderServe community programs, the Case Manager responds to all client requests, questions and concerns and reports frequently on the client's status and progress. Long-Term Home Health Care staff are available 24 hours a day, 7 days a week.

Telemedicine

“Telemedicine will allow our staff to detect early warning signs in a change of health,” said Daniel Reingold, President and CEO of The Hebrew Home. “It will help prevent chronic conditions from escalating into something more serious.”    

Telemedicine offers the latest in technology for residents who are homebound due to chronic health conditions or disabilities. Employing small, easy-to-use machines in the home, the Telemedicine system monitors daily vital health information electronically, thereby reducing unnecessary ER visits and hospitalizations.
For about $4 a day, ElderServe patients living at home can have their machines programmed to check vital signs such as blood pressure, heart rate, or glucose levels at set times throughout the day. The information can then be transmitted to an ElderServe nurse, who immediately monitors the results. If the reported information is outside of the normal range, the nurse can arrange for the necessary visit or medical treatment.

Short Term Stay

SHORT-TERM REHABILITATION

Rehab on the River is a comprehensive and interdisciplinary rehabilitation and post-acute program of physical therapy, occupational therapy, speech and language therapy that enables patients to achieve their maximum level of function and independence.  Our expert clinical staff, including physicians, nurses, dieticians and social workers, are trained specifically in care management and work closely with patients and families to enable patients to return home as soon as possible.  

We provide therapy services 365 days a year and patients may receive up to two to three hours of therapy a day depending upon their medical condition.  In addition, The Hebrew Home at Riverdale is the only skilled nursing facility in the tri-state area that has a warm water pool utilized for aquatic therapy, which is beneficial for a variety of medical conditions.

POST-ACUTE CARE FOR THE MEDICALLY COMPLEX
Patients requiring complex medical care can greatly benefit from a stay at the Hebrew Home.  We provide specialized services including wound care, respiratory care, pain management, intravenous therapy and post surgical recovery and rehabilitation.

We are unique in having a full time on-site medical staff available 24-hours a day to provide medical care to our patients.  In addition, we also have an on-site lab and on-site pharmacy enabling us to get test results and medications on the same day they are ordered.

WHO IS ELIGIBLE FOR POST-ACUTE CARE
Medicare recipients who require post-acute care subsequent to a hospital stay may be eligible for the Hebrew Home’s post-acute care program.  For managed care members, a prior hospital stay is often not required.

The following may require post-acute care:

  • Cardiac conditions (CHF, post-surgery)
  • Cerebral Vascular Accident (stroke) and other neurological conditions
  • Orthopedic conditions (post fractures, hip replacements, knee replacements)
  • Parkinson Disease and Movement Disorders

Special Needs

For those residents with memory care issues, the Hebrew Home offers several special  needs neighborhoods. These are for residents who need assistance both with activities of daily living and with orientation to place and time.

In addition, post-acute patients who have been discharged from the hospital and come to the Hebrew Home for rehabilitation and post-acute care may also come to special needs neighborhoods for a variety of reasons ranging from wound care to intravenous medications, before returning to the community.

Geriatric Care Management services provided by the Hebrew Home at Riverdale is a niche service that can aid in managing care for seniors who have no relative nearby to help in care or who who simply need a trained health care professional to provide  hands-on assistance.  We can be of assistance whether you live out-of-state or nearby. A certified geriatric care manager guides families through short term adjustments during changes in senior care levels or when on- going management and consistent monitoring are needed.

Geriatric care managers help navigate families through community and government services and assist clients in formulating a plan that will be the most effective in providing a safer and more secure environment.  We are there when you can’t be: In emergency situations, when someone is needed to oversee arranged services, coordinate care, and to help facilitate communication lines between familiy members and health care providers.

The Family Caregiver Resource Center in Yonkers, provides a one-stop resource center for information and assistance to family caregivers. In addition, counseling is also provided for long term care in the community setting by providing linkages with social adult day centers and other family health, social and legal service agencies.

Free In-Home assessments and referrals to other agencies in the community are available. In addition, education and support workshops are offered. Scholarship assistance for family caregiver respite programs are available as are Speakers Services, for corporations, faith-based agencies and service clubs.

The Resource Center and Clearinghouse services people in Central Westchester County (Scarsdale, Eastchester, Bronxville, Tuckahoe, Hartsdale, White Plains, Greenburg and the River Towns).

Elder Abuse Services

It is the mission of The Weinberg Center for Elder Abuse Prevention to provide emergency shelter for victims of elder abuse and to enhance public awareness and knowledge about elder abuse.

The Weinberg Center for Elder Abuse Prevention is the nation’s first comprehensive regional elder abuse shelter, serving eligible seniors who are 60-years and older.A unique model based within an existing long-term care facility, The Weinberg Center offers victims a full range of healthcare and supportive services including an emergency residential shelter and a coordinated system of care that provides a safe harbor, emotional support, psychological counseling, healthcare, legal advocacy and representation for victims of elder abuse.

Clients are encouraged to make full use of the Hebrew Home’s social, recreational and  educational programs, and to make use of the facility as a sanctuary from their previous environments. An interdisciplinary plan of care is developed for each client including a discharge plan, to the client’s home, if safe, or to alternative housing.
